Patient-Centered Care Transitions

The Massachusetts Senior Care Foundation hosts the Care Transitions Education Project (CTEP), an innovative workforce training strategy that equips frontline nurses and nursing students to lead and improve patient-centered care transitions.

Culture Change

The MA Culture Change Coalition is part of a national movement that is transforming nursing facilities into resident-centered communities. This philosophy nurtures the human spirit and promotes a living environment where older adults, people with disabilities, and their caregivers will thrive.

Strengthening Resident and Family Councils

Mass Senior Care participates in a one year learning collaborative to support Massachusetts nursing facilities in developing, enhancing, and sustaining resident and family councils, based on best practices and innovative approaches for working with residents and families.
